IMPORTANT NOTICE

In July 2006, the Colorado Legislature passed House Bill 06S-1009, which requires the Division of Registrations to verify that applicants for licensure be lawfully present in the United States. Any individual issued a license or registration by the Division with an effective date on or after January 1, 2007, must complete and sign an Affidavit of Eligibility. The Office of Inspector General (OIG) of the U.S. Department of Health and Human Services has the authority to prevent certain individuals and businesses from participating in federally-funded health care programs. The criminal convictions, sanctions, and other criteria that can result in an individual or business being excluded from such health care programs are outlined in this memorandum.
